NHS Hammersmith and Fulham Clinical Commissioning Group would like evaluate and, where appropriate, re-design our community Dermatology GP with Special Interest and Minor Surgery service. To support this work we would like to gauge individuals’ experience of the service through their completion of this questionnaire. It is very short and should only take you 10 minutes to complete.
