People with learning disabilities often have poorer physical and mental health than other people. However this doesn’t need to be the case. The Annual Health Check scheme is for adults and young people aged 14 or above with learning disabilities, who need more health support and who may otherwise have health conditions that go undetected. Sobus is a new Community Development Agency for Hammersmith & Fulham. It has been created through the merger of the Community and Voluntary Sector Association Hammersmith & Fulham (CaVSA) and the Fulham Community Partnership Trust (FCPT).
