Agenda

As you are already aware, we are seeking to improve our patients’ experience of patient transport across North West London. Following on from last year’s workshop we are holding another event for patients and service users to discuss how they will be assessed for patient transport services across North West London in the future.

We are inviting patient transport service users to join us on Thursday 29 June, 10.30am – 1.30pm at St  Paul’s Centre, Queen Caroline Street, Hammersmith, W6 9JP.

If you wish to take part, please register by Friday 16 June, via email Travel@nw.london.nhs.uk, phone: 0800 1777 990 or write to: FREEPOST: HEALTHIER NORTH WEST LONDON letting us know your full name, address, contact number, and email address. If you currently use patient transport, please let us know which hospital(s) you use. If you have any access or dietary requirements please let us know.
We will cover travel costs, or if you require patient transport due to your medical access needs this can be arranged for you.
Refreshments and lunch will be provided.
