The S&T outpatient transformation team are starting the process of reviewing some the outpatient service pathways to achieve the right specialise advice to patients at the right time and place. The Transformation Programme will be delivered over the next three years and seek to make considerable improvements in patient and staff experience and the quality of care offered to patients, while making the best use of available NHS resources.
To achieve these ambitious goals, the team are working together with all CCGs, provider Trusts, Primary Care and users to transform the way in which they understand, diagnose and manage care within available budgets. Core to the programme will be the development of pathways of care which encourage patients to be actively engaged in their own care from the decisions that individuals make about their lifestyle, decisions about their care and supporting individuals to self-manage conditions where this is clinically appropriate.

This programme will continue over the next three years, with the initial phase looking at five key clinical specialties below.

  • Cardiology
  • Trauma and Orthopaedics (MSK)
  • Dermatology
  • Gastroenterology
  • Gynaecology

As part of this initial phase, the team would like to invite some patient representatives who may have attended those outpatient departments (from any hospital in NWL) to participate in a number of workshops (the details are in the attached JD).
