Primary Care Networks (PCNs) involve GP practices, community services and local residents working in partnership to improve the health and wellbeing of communities. Link workers are key to this process. 

This link worker role is about people and places – supporting patients to become more independent, and building relationships within the local community, so we are looking for enthusiastic, motivated and energetic individuals who have a passion for people and want to make a difference to the community.

Family Friends is a Queen’s Award for Voluntary Service winning charity based in North Kensington which specialises in befriending and mentoring. We are recruiting a full-time Family & Volunteer Co-ordinator to join our team. The charity provides long-term practical and emotional help to disadvantaged families living in Kensington & Chelsea, Hammersmith & Fulham and South-East Brent through a network of 40-50 volunteers. Each volunteer supports either the parent or child in a family. We have a great record for ‘helping families to help themselves’. In this varied and exciting role, the Family & Volunteer Co-ordinator’s work includes:

  • Recruitment and training of volunteers
  • Introduction, supervision and support of volunteers
  • Assessment of children and parents needs via home visits
  • Monitoring and evaluating the services
  • Some evening and weekend work is required.

Standing Together Against Domestic Violence (STADV) coordinates the ground-breaking coordinated community response to domestic violence and abuse in London. As a pioneer of the first multi-agency group to implement safety measures for victims of domestic abuse, STADV seeks to appoint a dynamic MARAC Team Manager to operationally manage the work of its MARAC Coordination Team in West London.

The role will require you to coordinate one MARAC and to line manage four full time MARAC Coordinators. You will provide a high-quality management service delivery, performance and support to the ongoing development of the MARACs.

The post will be based in the STADV office with a degree of travelling within West London.

Salary:  £34,00 to £36,000 including London weighting depending on experience

Carers Cuppa Volunteer- Carers Network have launched a new project called Carers Cuppa in partnership with Charing Cross Hospital and Royal Brompton Hospital. Each hospital will have a designated “Carers Cuppa” table within the hospital cafés, offering a place to support unpaid carers of the sick and disabled.

Carers Cuppa Volunteers will reach out to hidden carers and highlight that the Carers Cuppa table is designated place to meet other unpaid carers in a similar position, whilst networking with hospital staff to promote the project to patients. The volunteer will also promote our services and facilitate referrals to Carers Network.

Community Outreach and Events Volunteer: Delivering information stands in GP Practices, promoting our services and volunteering opportunities at job fairs, and volunteer fairs. Other aspects of the role include delivering support groups or greeting and liaising with carers at social events. 

Community Link Worker- The Community Link Worker is an outreach-based role that will work closely with our Service Managers to travel tri-borough. The role of the Community Link Worker is to build relationships with new partner organisations to increase referrals to Carers Network, and to raise awareness of our services in the wider community.
