Community Cardio-Respiratory service – live from 1st April 2016

Imperial College Healthcare NHS Trust (ICHT) has been commissioned by H&F CCG to provide an innovative new integrated Community Cardiology and Respiratory service (hearts and lungs) across the borough of Hammersmith & Fulham. This new service commenced on the 1st April 2016, replacing the outgoing community Respiratory service run by CLCH.

The Community Cardiology and Respiratory Service is a multi-disciplinary service for all patients within the borough of Hammersmith and Fulham. The key principle of the service will be joint working, with the service operating as an integrated Cardiology and Respiratory service.  It will provide regular joint clinics to diagnose patients, multidisciplinary team case reviews, and joint or co-located community clinics at the same time to provide joined-up care for patients with breathlessness, and/or more than one heart or lung problem.  The service consists of consultants, specialist nurses, clinical psychologists, specialist physiotherapists, physiologists and administrators. They offer advice, consultation, assessment and management of heart and lung symptoms and diseases.
