2017 / 18 H&F CCG Grants Programme

Hammersmith and Fulham Clinical Commissioning Group (H&F CCG) has commissioned Sobus to manage a Health and Wellbeing grants programme for voluntary and community sector organisations to deliver projects in Hammersmith and Fulham.

Here is a list of the successful applicants

6 The Health Forum (aka BME Health Forum)  £   6,330.00 Project to understand and improve access to NHS services for BME communities not fluent in English
7 Women’s Association For African Networking And Development (WAND) UK  £ 12,500.00 Raising awareness of cancer particularly BME women from deprved communities
8 Open Age  £ 12,500.00 Support for older carers supporting others including those with physical, mental health & learning disabilities
9 DanceWest (CIC)  £ 10,500.00 Dance classes for older people 65+ in care homes, sheltered housing and health & wellbeing centres
10 24. HCGA (Healthy Community Gardening Activities)  £   8,840.00 Horticultural activities and healthy eating for older people and carers
11 33. deafPLUS  £   7,500.00 Activities for deaf and hard of hearing adults to tackle social isolation and health inequalities
12 20. Elgin Close Resource Centre (part of Notting Hill Housing)  £   8,124.00 Project to stimulate activity physical, mental and social in people with later stage dementia

Sobus ran a grants programme on behalf of H&F CCG between 1st April 2016 – 31st March 2017. Applications were open until 12th February 2016.

We had 43 applications in total amounting to over £460,000.
