The British Red Cross provides time limited support in the home for people after an accident or illness, giving them the confidence to continue their daily lives and regain independence. Going home after a stay in hospital can often be an unsettling experience, British Red Cross helps people over the age of 60 living in Kensington & Chelsea, Westminster, Hammersmith & Fulham to settle back into their homes, linking them back into the community with relevant services & giving them the confidence to continue their daily lives. British Red Cross assist in safe and efficient discharges & provide practical and emotional support to ensure patients are more resilient to readmission.

After an initial assessment, British Red Crosswill carry out up to 4 home visits over a 4 week period.

  • accompaniment to and from outpatient and GP appointments
  • friendly chats and company
  • prescription collection
  • light food and drink preparation
  • collecting or assisting with shopping
  • assistance with light household tasks
  • help with paying household bills and form filling
  • help to access benefits and entitlements
  • going for walks or using public transport
  • telephone support
